S.N.A.P. - Special Needs Health Alert Program   The Special Needs Alert Program, "SNAP" was developed by the Department of Health and Human Services to assist you in caring for your child in the event of an emergency.  If your child has a chronic health issue such as Asthma, Diabetes, or a medication/food alergy, your child's medical information will be on file with the EMS system in the event that there is an emergency at home or at school.  This program assists the EMS team in better preparing for emergency care for your child by having pertinent information available to them prior to their arrival at the scene of an emergency to care for your child.

 

Regulations and Policies

Policy for Early Dismissal from School due to Illness

All students MUST come to the Nurse's Office and be assessed by the School Nurse PRIOR to a parent being called to pick up a student.  No student may use a cellphone, email, or text message to contact a parent to request that they be picked up.  The phone call must be initiated by the School Nurse or the absence will be considered UNEXCUSED.

Medication Policy

No student is permitted to carry medication with them of any kind during the school day with the exception of Asthma Inhalers or EpiPens for severe allergic reactions.  Students are only permitted to carry and use an Asthmatic Quick Relief Inhaler or Autoinjectable Epinephrine once the Self-Administer Medication Form signed by student, parent, and physician is on file with the School Nurse.  An Asthma and Allergy Action plan must also be completed and on file.  These forms will be sent home over the summer for 10th-12th graders in the beginning of the school year for incoming 9th graders.  YOUR STUDENT WILL NOT BE PERMITTED TO ATTEND FIELD TRIPS UNLESS THE SELF-ADMINISTER MEDICATION FORM AND ASTHMA OR ALLERGY ACTION PLANS ARE ON FILE IN THE NURSE'S OFFICE.

All prescription and over-the-counter medication sent into school must be in properly labeled bottles with correct child's name, drug name, exact dose, and time to be dispensed.  A proper parent note MUST include the child's name, drug name, dosage, drug route, exact time(s) to be given, exact date(s) to be given, reason for being given, allergies, and a number where parent can be reached.  You may also use the request for prescription/OTC medication form found under "Forms.*"

*Please note that if the medication that you send in for your child does not meet ALL of the above criteria for labeling and proper parent note, the medication WILL NOT BE GIVEN.  The medication will be sent home with your child.  NO exceptions will be made.  We cannot accept telephone permission to administer medications.  These requirements are a State of Delaware law.

 

Delaware Regulations 804, 805, and 806

Regulation 804:  Immunizations:

  • 5 or more doses of DTaP, DTP, or TD vaccine  (unless 4th dose was given after the 4th birthday)
  • 4 doses of IPV or OPV (unless the 3rd dose was given after the 4th birthday)
  • 2 doses of measles, mumps, and rubella vaccine (first does after the age of 12 months, second dose after the 4th birthday)
  • 3 doses of Hepatitis B vaccine
  • 2 doses of Varicella or a written disease history by a licensed healthcare provider

Regulation 805:  Tuberculosis:

New school enterers shall show proof of Tuberculin Screening results including either results from the Mantoux Tuberculin test or the results of a Tuberculosis Risk Assessment.

Regulation 815:  Physical Examination and Screening:

All public school students shall have a physical examination that has been administered by a licensed Medical Physician, Nurse Practitioner, or Physician's Assistant.  This may be a standard physical, or if your child is playing sports, the DIAA sports physical will also be accepted.  Within fourteen calendar days after notification of the requirement for a physical examination, new enterers shall have received a physical examination or shall have a documented appointment with a licensed health care provider for a physical examination.
